Hi doctor,

I am having pain on the right side of the rib cage just 6 to 7 cm away from the armpit. It is like tightening pain or stabbing pain and sometimes burning sensation. It seems to be related to acid reflux and what I do is drink 2 to 3 glass of warm water by rubbing fingers on the backside of the tongue and spitting that water out. After that, I took Pantoprazole and slowly got relief. But this thing is usually occurring twice a month. I have had treatment of gestational trophoblastic neoplasia and got three cycles of Methotrexate. At that time, the situation was worse, but it came to be a bit normal after treatment. Currently, I take DGL Deglycyrrhizinated licorice root extract. Please help me.

Thank you for writing, and I understand your concern.

Yes, it can be due to acidity or gastroesophageal reflux, which occurs due to the stomach's overproduction of acid. The other reason could be respiratory tract infection. And it is sometimes associated with the liver. So first of all, I would suggest you do a chest x-ray and liver function test.

To check if both systems are functioning correctly or there is some infection. After ruling out, I can say that it is due to acidity or not. Please do the test, and you can send me the reports. I will check and let you know further. You can continue Pantoprazole 40 mg once a day before a meal for five days. It will help you.

  • Did you have any respiratory tract infection recently?
  • Is there any chest tightness or difficulty breathing?

Hi doctor,

I have no chest tightness or difficulty breathing. As far as the liver is concerned, I have had chemotherapy sessions, and the last session ended two months back. I tested for Alanine aminotransferase and Aspartate aminotransferase, and it was near 245 and 183. The doctor said that it is evident when you are on chemo (Methotrexate), the liver enzymes elevate but will come back to normal naturally after chemotherapy stops. So is it because of that? How long does it take to go back to normal? Sometimes I feel stiffness in the area near the collarbones of the sides of the neck or the center of the neck as if something is there, but it rarely happens for a few minutes.

The pain can be due to elevated liver enzymes only. Can you please tell me when did you start the chemotherapy? How many sessions did you take? And for what? Please tell me about your previous disease. By now, it has to be reduced so you can do liver function test to check liver enzymes. If still high, they have to be treated accordingly.

Thank you for your reply.

The first session was before three months. Methotrexate 25 mg per day that 125 mg for five days. The second session was before two months. And the third and the last session was a month back. It was for gestational trophoblastic neoplasia stage 1 low risk.
