Hello doctor,

I had a sexual encounter with CSW(Community Sex Worker) which was protected and I have no symptoms. After six months I have another sexual encounter with CSW which was also protected. After a month a weird thread-like thing started appearing in my penis and it got disappeared in a few days. Now, again I get this same thread appearance and it keeps multiplying in different areas in the penis. Also, I am having intercourse with my wife (unprotected). She has no symptoms and she is breastfeeding. Is there any possible risk for her or for the baby. Let me know if anything else is required. Also, I think it is high time to have a full STD (Sexually Transmitted Diseases) test. I had one last year and it was clear.

I have seen the photos sent by you. (attachment removed to protect patient identity). Yes, you need to go for complete STD (sexually transmitted disease) panel. You are suffering from one of the STD known as genital warts, more specifically filiform type genital warts. Electrofulguration is the treatment of choice in these cases.

Can you please prescribe me the list of tests to be taken. Is there any other treatment like tablets or gels? How do I save my wife and baby? I am embarrassed for the treatment if you could help me it would be great.

Go for HIV fourth-generation test, ELISA (enzyme-linked immuno-sorbent assay), VDRL (venereal disease research laboratory), HSV IgG and IgM (herpes simplex virus). I would advise you to refrain from unprotected sex with your wife as the genital wart is a communicable disease and would be very difficult to treat in females because of complex genitalia anatomy. Tablets or gel are very less effective nevertheless you can use Podofilox gel (Podophyllotoxin), every alternate day only over the lesion i.e. it should not touch the normal skin. Keep the area open after application in order to get it absorbed without spreading to the normal skin. I hope this helps.
