Hello doctor,

I am 20 years old, and I have a problem with jock itch. Recently, I noticed some small black marks on the inner glans of my penis. I do not know the reason for this, and I am very worried about it. I do not understand why I am experiencing so many skin problems. I have never had any sexual contact with anyone. Is this all because of masturbation, and is it curable? I do not know how long I have had this problem; I only noticed the black marks about a week ago. It is not causing any pain, but I have had jock itch for a long time, and I feel embarrassed to talk to anyone about it. The jock itch is similar in color to my skin, so I never paid much attention to it. However, when I noticed it was spreading very slowly, I told my parents about it. Regarding my current medical complaint, I had ringworm five years ago, although it is not the same issue. Could the change in color on the inner glans of my penis be related to the jock itch? Is there any way to remove these black marks? Are they harmful or normal? I am really worried about this. Please help me.

I have reviewed your medical history and understand your concerns.

I have reviewed the attached image (attachment removed to protect the patient’s identity). It seems you have tinea cruris with balanitis. Tinea cruris is a fungal infection of the groin, and balanitis is inflammation of the glans penis. Follow certain general measures which include:

  1. Do not keep the area wet at all.
  2. Use clotrimazole powder frequently to keep it dry.
  3. Avoid itching and do not scratch it.
  4. Change your undergarments regularly. Avoid excessive sweating and wet clothes.
  5. Do not apply any general or steroid creams like Betnovate.
  6. Do not stop the medicine once you cure it. Continue taking the medicine for a long time to completely eliminate the fungus.
  7. Check for diabetes and thyroid issues if it recurs even after proper treatment.
  8. Treatment part: Apply 2 percent Ketoconazole cream (KZ Cream) twice daily and take tablet Terbinafine 250 mg for three weeks after consulting your general practitioner. Over the glans, apply Ketoconazole plus Fucidin (Fusidic acid) cream twice daily for one month.
