Hello doctor,

I underwent a circumcision three weeks ago. The stitches have almost totally dissolved, and I have gotten used to the sensitivity pretty well, but the swelling did not go down in one spot. At the two-week appointment, my doctor said the swelling was actually a "blood clot" of sorts, but not one that travels into the heart or lungs. I am still bleeding a bit, and he said eventually it will just bleed all that blood out. Sometimes, I squeeze it to try and get the blood out; it does not hurt at all. It is not too hard either, considering I can squeeze it a bit. Should I be concerned? I think I read something online about how the slow drainage might be linked to lymph nodes, which I have had swell up a few times when I got a cold or flu. What to do?

I am sorry to hear about the issues you are facing. From the photo you uploaded, I can see that you have a blood clot or haematoma on the bottom side of your penis (attachments removed to protect the patient’s identity). This can sometimes happen as a complication of circumcision surgery. The blood clot will not dislodge and travel to your heart, but there is a risk of infection if it stays there for too long. However, I suggest you to follow a few instructions; Take a warm bath with rock salt every day. This will help reduce inflammation. Keep the area clean to prevent any infection. You can try taking the tablet Chymoral Forte (Trypsin and Chymotrypsin), one tablet three times a day for ten days. Please note that the brand name may be different in some countries, and I suggest you consult a specialist, talk with them, and take the medications with their consent. Alternatively, you can take the tablet Zerodol SP (Aceclofenac and Serratiopeptidase), one tablet twice a day for 10 days. Avoid squeezing the clot too hard, as it may cause damage to the sutured area. Usually, with time, the clot should subside on its own. However, if it does not improve, I suggest consulting your surgeon to have the blood clot aspirated using a white bore needle. It is important to avoid sexual intercourse for at least a month to allow proper healing.
