Hello doctor,

I am 38 years old, and have a history of endometriosis. 17 years back I underwent the removal of an ovarian torsion cyst of 10 cm by open surgery. My ovaries were preserved and I had no complaints except some cysts since then. But now when I want to conceive I have complications and found three complex cysts of 8, 7, and 6 cm with 300 cc fluid in the right ovary with mural nodules and debris. I am feeling a dull ache on the right side, a heavy bulky ovary, and a hydrosalpinx in the left tube. What is the treatment to be followed? Is there a need to remove the ovary or hormone treatment after cyst removal and salphinography? Kindly enlighten. My Anti-Mullerian hormone as of now is 4.04 ng/mL. Can I ever conceive?

  3. Have you undergone a laparoscopy?
  4. Have you received ovulation induction ever?
  5. Have your tried IVF (invitro fertilization)?

According to the size of your ovarian cysts, you will have to undergo laparoscopy with cyst removal. Laparoscopy is the gold standard for endometriosis. Immediately after laparoscopy, you will have to conceive. Given your age, I would suggest you directly go for IVF without wasting any more time. And yes, you can definitely conceive.
