Does Gonal-F with Menopur increase the egg quality?

Hello doctor,

I have two children already, and thankfully both conceived easily. My older son has a chromosomal anomaly. Although we are not sure that this was a hereditary problem, we wanted to do IVF with genetic testing to grow our family further, as it would be tough for us to have two children with a disability. I was surprised to find out that my AMH is relatively low, and my AFC was 10. So the clinic was not overly concerned. My husband is 47 years old. I was given 300 IU of Menopur and Cetrotide. After four days of stimulation, I had eight follicles. Two were 16 mm, three were 14 mm, and the other three were 10 mm. I continued on the same protocol for another six days. At egg retrieval, they got seven eggs, but only three were mature and suitable for ICSI. All three fertilized, but only two made it to day three, and they were of poor quality. They want me to go back and repeat the same procedure one more time. But I am not sure if it is helpful. This time they are putting me on 150 Gonal-F and 150 Menopur. Does this sound like a good plan? What do you think was the reason for the failed round?

Thanks for writing. I have gone through your history in detail, and I fully understand your concerns. It is better to go for IVF (in-vitro fertilization) with your whole history as it has a higher chance of healthy pregnancy. Kindly give some breaks and go for another cycle of IVF with a high dose of medicine to improve the egg size to more than 23 mm.

In my opinion, your first cycle failure could be due to an egg quality issue, and also IVF procedure has only 20 to 21 % chances of pregnancy. Every cycle is not 100 % effective. With your details, follicular size reaches more than 23 mm, but you did not conceive. That means it did not rupture. So, the effects of medicine dosing in failure of the procedure are on the small lower side.
