Hello doctor,

I am having a severe indigestion problem. I am a 26-year-old male. Three years ago, I have used a strict diet in order to lose a big amount of weight in which I believe it affected me and affected my digestion. Since then I only have one big meal a day which is mostly dinner and I bloat myself after eating which most of the time leads me to throw up my food. During the day, I do not crave anything but coffee. I gained weight and I do not mind it but I want advice regarding my issue. Do you suggest any indigestion enzyme (other than changing my lifestyle)?

How frequently do you vomit per day? Is that related to any specific kind of meal? Do you have diarrhea as well? How had you lost your weight? Which specific dietary therapy? How much weight had you lost and how much have you regained now?

Have you ever had any medication or investigation for your illness? Do you have a history of any other major illness? Do you have any surgical history?

If you have not tried any medication, then I will initially start a medication and get some investigations done. If it does not work then I might advise another investigation.

Investigations to be done

Barium meal and follow through. Complete blood count. ALT (Alanine Aminotransaminase). Ultrasound abdomen.

Differential diagnosis

SMA (superior mesenteric artery) syndrome?

Treatment plan

Tablet Levosulpiride 25 mg thrice a day before meals for one month, capsule Omeprazole 40 mg once daily 30 minutes before breakfast for one month.

Preventive measures

Daily exercise. Avoid fatty meals, carbonated drinks and junk food. Try to avoid coffee and see if it helps or not.

I have not tried any medication yet. Regarding the vomit, it happens when I am really full and I cannot stand myself. No diarrhea and it does not apply on a specific meal. It is almost like whenever I eat a meal and get full and I want to vomit so I can feel relief.

Regarding losing weight, I have not followed a certain one, I used to eat clean food twice a day and exercise. I have lost almost 66.1lbs and now I gained like 11-12 lbs back. I have not had any medication. No illness nor surgery.

Regarding COVID-19, I cannot make any test any time soon but what could you suggest to ease my situation?

Please start the prescribed medications and do the advised investigations in first chance. Meanwhile, please take small and frequent meals (preferably soft diet). But in case, you do not get relief then you will have to go for investigations.
