Hi doctor,

My child's details are as follows: Basal age three years, mental age four years six months, ceiling age eight years, IQ-74, SQ-86, CARS 21, Connors abbreviated rating scale 19. How will be his future in terms of academic and social?

Thank you for taking help from a psychiatrist. Your child has a borderline intellectual deficiency with ADHD (attention deficit hypertensive disorder) and epilepsy. I am sorry to say that your child needs lots of care. It is quite difficult for him to compete with today's children. You can teach him daily. Your child will take extra time to learn. Just example, if a normal child learns by seeing others or learning in one day, then your child will take two days to learn. The important part is that you have to teach him. If you do not teach him, then he will never learn. For epilepsy, you have to be sure that he does not get any episodes of convulsions. Every episode will affect his intelligence power. If he continues to get convulsion, then he will get more and more IQ (intelligence quotient) and learning problems. So some non pharmacology ways to prevent further episodes are, never keep hungry. Never skip his anti-epilepsy medication (Valproate). Give him daily during fixed time. Never do sleep deprivation. Make sure for adequate sleep. Whenever he gets a common cold or any illness, do immediate treatment to prevent high fever. If his body temperature rises beyond 101 Fahrenheit, then he has a chance of convulsion. There is no medication that can make your child intelligent so stay away from fake medication. You just have to train him to become independent. ADHD (attention deficit hypertensive disorder) medication can help him, but it will increase the chances of convulsion and irritability.

Do not get angry or beat your child in any situation. It is very difficult for you to manage a child, but it is not going to help you. Whenever he gets angry then try to calm his irritability, and divert him in another way. Do not get angry with him. The prognosis is not so good, but you can make it fair by teaching him or training him to be more independent. Parents can do anything. It is in your hand. Medication will support him. Many borderline intellectually deficient people live an independent life with minimum supervision.
