Hello doctor,

I am a 24-year-old male, actively involved in homosexual intercourse (anal) for many years, both safe and unsafe, with multiple different partners. However, a few months back, after safe anal intercourse (using condoms), I felt an itch on my anus. I noticed very minute white scaly bumps on my anus, and sometimes I feel they regress, and other times they increase. They are not big, they are minute, and now I have a weird scratch on my anus. Please help.

I can understand your concern.

I have checked the picture (attachment hidden to protect the identity of the patient); if there was unprotected sex - one needs to think of sexually transmitted infections like:

1. TPHA- Treponema palladium hemagglutination.

2. HPV (human papilloma virus) DNA PCR test.

3. Herpes simplex virus (HSV) one and two IgG.

4. Herpes simplex virus (HSV) one and two IgM, serum.

5. Chlamydia trachomatis IgG.

6. HIV (human immunodeficiency virus) one and two antibodies, screening test.

7. HbsAg (Hepatitis B surface antigen).

8. Urine complete.

If not, it looks like an injury, make sure to use lubricants. If there is itching, you need to think of fungal infections too - use ointment Candid B (Beclomethasone and Clotrimazole) for local application and for itching – tablet Cetirizine once a night. Make sure you wash your genitalia twice a day, morning and night, and use normal water, no cold or hot water. Do not use heavy pH soap.
