Hello doctor,

I am having a problem with my genital skin. A few bumps appeared some days ago, which were very itchy. I scratched them, and now they are bigger and burn. I cannot move or perform my job because it hurts after I scratch. I initially used Sodium Fusidate, stopped them, and started using Hydrocortisone cream, but the condition has not resolved. The bumps are getting bigger with increased itchiness and inflammation, which has left a scar on the skin. Please help.

I have read the query, and I can understand your concern.

You have tinea cruris, according to the clinical picture you sent (attachment removed to protect the patient’s identity). Tinea cruris is a fungal infection of the groin. I suggest you follow these measures:

  1. Do not keep the area wet at all.

  2. To maintain the area's dryness, apply Clotrimazole powder often.

  3. Avoid itching, and do not scratch it.

  4. Change the undergarments regularly.

  5. Avoid excessive sweating and wet clothes.

  6. Do not apply any general or steroid creams like Betnovate (Betamethasone), Surfaz- SN ointment (Clotrimazole, Beclometasone, Neomycin), Quadriderm RF cream (Clotrimazole, Beclometasone, Neomycin), Panderm NM cream (Clobetasol, Miconazole, Neomycin), Hydrocortisone, Fourderm cream (Chlorhexidine Gluconate, Clobetasol, Miconazole, Neomycin).

Keep taking the medication even after you feel better to eradicate the fungal infection. Check diabetes and thyroid if it is recurrent even after proper treatment. The disease is not life-threatening and has no complications, but is contagious. The aforementioned measures can easily prevent it. Fungus always needs humidity and a wet area to penetrate, so keep it dry. I suggest the following treatment: If fungal infection does not respond to Terbinafine or Fluconazole, it may be because of resistance development. You can take the tablet Itraconazole 200 mg twice daily and KZ (Ketoconazole) cream twice for local application for one month. Consult a specialist doctor, talk to them, and then take the medication.
