Hello doctor,

The Tinea cruris has been spreading​ in my underwear area. It is very itchy. I am using candid B cream but after a few hours, it has started again. Please suggest some curable tricks and suggest some medicines which are available in all medicine shops.

I have read your query and understand your concern. It looks like tinea cruris. Tinea cruris is a fungal infection of the groin, I suggest you follow certain general measures

1. Do not keep the area wet at all. Use Clotrimazole (Zinc oxide and Aerosil) powder frequently to keep it dry.

2. Avoid itching and do not scratch it. Change the undergarments regularly. Avoid excessive sweating and wet clothes.

3. Do not apply any general or steroid creams like Betnovate, Surfaz ointment [Clotrimazole (1% W/W), Betamethasone (0.05% W/W), Tolnaftate (1% W/W), Neomycin (0.5% W/W)], or Candid B.

4. Do not stop the medicine once you are cured of it. Continue medicine for a long time to completely kill the fungus.

5. Check diabetes and thyroid. It is really recurrent even after proper treatment.

6. It can be transferred. However, it is not a dread disease (life-threatening) and almost has nil complications. It can be easily prevented by the above-mentioned measures.

7. Fungus always needs humidity and a wet area to penetrate. So, just do not give it a chance by keeping it dry.

Treatment part:

• This time trend for fungal infection is that it is not responding to Terbinafine (Terbina force) or Fluconazole may be because of resistant development.

• You can take Tablet Itraconazole (Hypromellose and Polyethylene glycol) 200 mg twice a day and KZ (Ketoconazole- Polysorbate 60 and Propylene glycol) cream twice for local application for one month.

All medicines should be used after consulting your general physician or nearby dermatologist. I hope you find this useful.
