Hello doctor,

I am about 43 years old, and I am planning to have kids, but unable to hold an erection. I do get an erection but by the time I go for penetration, if there is any slight distraction, it goes down and I am unable to get it again. I have also noticed that my mood does not last for long, maybe due to stress. While performing, I get very exhausted, my chest feels heavy, and I get tired instantly. I have had asthma since childhood, but then in my 20s, I was able to have an erection and perform the act well. But from the age of 32 and later, I am not able to hold my mood and erection. Also, I do feel like having sex, and the urge is there, but when it comes to performance, it is just not happening. Sometimes when I see porn, I have a very good erection that lasts long. Sex is causing me to get exhausted very fast and resulting in softening of my penis within five minutes. Please help.

According to your complaints, you may be suffering from erectile dysfunction (ED). Reasons for erectile dysfunction include:

  1. Stress, work, or family-related duties.

  2. Depression.

  3. Varicocele.

  4. Fear of failure - Having one episode of ED once may make you feel you will end up with ED.

  5. Low self-esteem (chronic masturbation).

Based on the above message, I have keyed in my reply below each point:

Q1: Stress due to work or family-related, night duties. Plan for regular exercise and yoga.

Ans: I have stress. I am divorced, and this is my second marriage. A lot of issues with my family and finances. My dad passed away last month. I have no night duty shift.

Q2: Depression.

Ans: Maybe it is a reason as I am constantly worried and not happy with myself.

Q3: Varicocele – ultrasound scrotum can help diagnose.

Ans: How will this help?

Q4: Fear of failure - having one episode of ED once may make you feel you will end up with ED.

Ans: I get too exhausted. That also might be the reason.

Q5: Low self-esteem (chronic masturbation).

Ans: I used to do this, but I stopped and do not find it helping.

Q6: Homosexuality – not attracted to same-sex.

Ans: I am not homosexual. I am clear about it.

Q7: Religious constraints.

Ans: I have no constraints.

Q8: Smoking and alcohol.

Ans: I am not a smoker and do occasional drinking.

Q9: Long-term illness – cardiac disease, thyroid disease, kidney disease, or any other disease with long-term medication.

Ans: I am asthmatic. I have knee pain and am unable to do exercise. I am also very demotivated with my life and feel very depressed.

Q10: Simple vitamin deficiency.

Ans: I am taking vitamin B12 tablets.

Q11: Negative attitude towards your partner.

Ans: No, I really love my wife.

Q12: Hormonal issue – thyroid issue, low testosterone, increased prolactin.

Ans: No drug usage.

Q14: Diabetes and hypertension.

Ans: I have no diabetes but high cholesterol.
