From the history, it seems you have developed lichen sclerosis et atrophicus. In this condition, discoloration and tightness occur.
Since when do you have this issue? Is it spreading? I need to see the discoloration so that I can assess.

Tightness or abnormal discoloration is not applicable except for the slight whitish color. I would suggest you take a urologist appointment for this and changing shape and tightness. He will be able to help you in a better way.
As there are no depigmented white areas, making a lichen sclerosis diagnosis is difficult without personal examination.
Apply Tacroz (Tacrolimus) cream 0.03 % at night in a small quantity on the white area for one month.
