Hello doctor,
A spot appeared on the skin of my penis. Seven months later, this stain spread and my skin began to become more sensitive. Then I used a cream with corticoid (Lotrimin) and it got worse because now my skin has shrunk and lost elasticity. What should I do? Is the corticosteroid effect reversible?

I can understand your concern.
I have noted your concern and I have viewed the images (attachment removed to protect patient identity). I think you have a condition known as genital LSeA (lichen sclerosis et atrophicus). LSeA is characterized by atrophy which manifests as tightening of the prepuce and the skin appears depigmented. There is itching. I do not think this is due to corticosteroids, especially a mild steroid like Lotrimin (Clotrimazole). In fact, potent corticosteroids are one of the treatment options for genital LSeA.

Thank you for the reply.
I thought it was the cream, because before it was not tightening. Only, when I tried to treat it, it began to tighten. So, what can I use to try to treat it? I used Lotrimin for 15 days and did not see improvement.

I can understand your concern.
I would suggest you use a more potent steroid like either Mometasone Furoate 0.1 % cream or Betamethasone valerate 0.1 % cream or a nonsteroid alternative like Tacrolimus 0.1 % ointment, twice daily for a few weeks. However, if these do not work, then circumcision is an effective option.
