Hi doctor,

I am a female, who is going to be 18 years old this year. So I have been sick (flu, fever, sore throat, and headache) since the last 20 days and I went to visit the doctor after five days. The doctor then prescribed me Lozenges, Paracetamol, Cetirizine and Amoxicillin. The diagnosis I received was acute respiratory infection. I also had a COVID-19 swab test which came out negative. I finished all my medicine and I still had fever (37.6° to 38.3°). I developed a slight cough so I just took some cough medicine by myself. However two days back, I had bad runny and blocked nose and muscle aches, which is persistent until now. My appetite has not been great as well, ever since I was sick. I only ate half a meal a day as I was very full just after eating half a meal. I only ate one slice of bread during dinner when I was taking medication. I also have been feeling nauseous the past week. Can you advice me on what should I do next? Because I seem to not be getting better.

I can well understand that you are worried, and I am saddened because you feel so ill. I will do everything I can to help you. As I could learn from your detailed descriptions, you were already on quite some medical drugs. Among them, and that is essential information, also an antibiotic, in your case, Amoxicillin. You also mention that you had a COVID-19 test and that it was negative. The problem is that these tests may be very accurate in theory, but they are prone to show false results in clinical reality. Therefore many countries test patients twice. And since your symptoms are very consistent with what we see in COVID-19, I strongly recommend repeating the test. In case the second test for COVID-19 should also be negative, your local doctor is well-advised to order a comprehensive test panel to check if you have a bacterial infection and which antibiotic is the most effective one in your case. Until you get the test results and given the fact that you are still showing symptoms, I will recommend you some medication that you can take until you get the test results. Treatment plan

Until test results, maximum of ten days: Aspirin 500 mg, three times per day (if tolerated), Famotidine 40 mg, three times per day. If not available, Gaviscon (Aluminium hydroxide and magnesium Carbonate) liquid or a similar antacid medication, three times per day, one hour apart from all other medications. Bromelain 50-100 mg, three times per day. Dextromethorphan 30 mg, three times per day. Vitamin D 1000 IU, three times per day. Zinc (aspartate or orotate) 50 mg, three times per day. N-Acetylcysteine (NAC) 600 mg, one time per day.

Preventive measures

Strict hygiene.

Regarding follow up

Follow-up after five days.
