Hello doctor,

I was diagnosed with adenocarcinoma lung stage 4 for more than four years now. Thereafter regular treatment as per the treatment sequence attached. I am taking Tecentriq 1200, Beveconumab 800, Genexol PM 300, and Cisplatin 100 mg for six cycles every 21 days. Despite this, the disease is in progression (copy of the last PET scan attached). I have also attached the histopathology report. Kindly suggest your opinion.

I read your query and can understand your concerns.

It does happen in stage four of lung cancer. Cancer cells become resistant after 3 to 6 cycles. We need to move to the next line of chemotherapy. It is the standard protocol. Tecentriq (Atezolizumab) and Genexol (Paclitaxel) should not be given together. You can go for Atezolizumab, Bevacizumab, and Cisplatin for four cycles, if these agents are not given earlier and repeat the scan after four cycles. If no progression, should plan another four cycles.
