Hello doctor,

I am a 22-year-old woman and am not on any medications except some Ibuprofen to get rid of a persistent headache. I have been feeling bad for more than a month now with no signs of a full recovery anytime soon. It started with simple fatigue, which I found odd. That developed into mild cold-type symptoms for the next 3 days (an occasional cough and a bit of a runny nose). By this month's start, I suddenly became extremely fatigued, and that same night, I got a horrible fever. That disappeared the next day, but I remained bed-bound until the weekend. After 3 to 4 days, I felt strong enough to leave the bed and do a few things, but since then, since then I have had daily headaches that come and go, forehead pain, an ever-increasing blocked nose, insomnia, and cough. Is this sinusitis, and if so, does it sound viral or bacterial? Do you recommend seeking antibiotics? Please suggest.

Regarding your symptoms, you have the following diagnosis: acute malaria with possible typhoid complications. Please do a malaria and typhoid test and please take the following:

  1. Tablet Acetaminophen 500 mg thrice daily.
  2. Tablet Rimegepant 75 mg once daily.
  3. Capsule Piroxicam 40 mg once daily.
  4. Tablet Artemether 100 mg twice daily.
  5. Capsule co-Amoxiclav 625 mg twice daily.

Consult your physician and take the medicines only with their consent. If your symptoms persist, schedule another appointment for follow-up.
