Hi doctor,
My sister-in-law is 3 months pregnant. Three days ago, during her antenatal scan, we came to know that it is a monozygotic twin pregnancy. We have a few questions. How to know that it is not a conjoined twin? I have attached the scan reports. Please let us know whether the growth is fine. It was found to be a twin only after 3 months. Will it cause any major difference in their growth? Also, she is a short person and 83 lbs weight. Also, she lost 6 lbs in the last three months due to pregnancy sickness. Will it lead to premature delivery or C-section? She does not have any health issue such as BP, thyroid, bleeding disorder, diabetes, etc. This is her pregnancy and I have attached the scan reports for your reference. Please guide us doctor.

Firstly, my hearty congratulations for twin pregnancy.
I have gone through the reports (attachment removed to protect patient identity) and they are perfectly fine. There are no signs of conjoined twins. NT (nuchal translucency) is normal and both nasal bones are seen. Heartbeat and growth is perfectly corresponding to the dates. Placenta has no problem and doppler flows are normal.Though twin pregnancy has to be looked a little carefully, but not all twin pregnancies have complications. A little extra care, repeated hospital visit and more frequent scans will definitely help you take necessary precautions. It is too very early to say things in detail, but as far as the scan goes it is perfectly fine. However, in the eventual scan all the details will be revealed.
Regarding growth of the twins, the grow scan will reveal the details and at this point of time the scan is perfect. Regarding mother's weight, usually in the initial three months most of the pregnant women loses weight and is not a matter of concern, until she has heavy vomiting. Twin pregnancy has many risk factors, including prematurity, but regular gynecologist visits, proper medications and health care measures will take care of the problem. Regarding C-section, most of the twin pregnancy will end up with a cesarean section, mainly because of the baby's outcome rather than a complication. Twin pregnancy comes with double responsibility, but not double stress. Just take care of her. Regular gynecologist visits, regular scan and tests will help any type of complications. You need not have to take so much pressure, as the pregnancy at this point is perfect and healthy. Ask her to continue with Iron and Calcium supplements.
