Hi doctor,

My girlfriend has piles. She gets blood along with stool. She is 22 years old. How to get it cured? How to get some relief from this? Is piles operation a complicated one? How many days should she need to take rest after this operation? Is there any restriction in food?

Welcome to icliniq.com. We are here to help you. Blood in stools can be from upper gastrointestinal (UGI) or from lower gastrointestinal (LGI). The dark altered blood is from upper GI bleed and fresh blood is mostly from the lower GI tract. We need to evaluate various possibilities that can cause such bleeding where the most common being hemorrhoids or piles, inflammatory bowel disease and acute dysentery. In such scenario, I would advise you to see your doctor, who will perform a proctoscopy in his office. If any evidence of piles is found, then she would be treated accordingly. If he feels suspicious of inflammatory bowel disease, then he will post her for a colonoscopy. Also, important here is that if she is having cramps, then this might be some infection or dysentery. It is not necessary that it will always be associated with loose stools. In such scenario, she would require antibiotics like Norfloxacin and Tinidazole. In case of piles, following are the answers to your questions. Medical management is done with stool softeners, local analgesic and antibiotic creams. If surgery required, then it depends on the extent of the piles. Surgery can be sclerosing injection into the piles and may even go up to resection of piles known as hemorrhoidectomy. Yes, oily and spicy food should be avoided.
