Hello doctor,

I am a 33-year-old female and I have been trying to get pregnant for the last nine months. I have irregular periods and sometimes I bleed up to six weeks but the doctors do not know why this happens. I am currently on Tranexamic acid tablets because I have been bleeding for two weeks (brown or pink) but the tablets do not seem to be helping. My AMH levels have increased from 7 to 26 over the last six months but my OEST levels have dropped to 85. I have been off the pill for over two years now.

You should be concerned about your fertility at this age. But surprisingly as your age is advancing your AMH level has increased which means the reserve of eggs in the ovaries is quite high. This is a good sign. This usually happens in polycystic ovaries. Since ultrasound is not showing any evidence of the increased volume of ovaries or cause of irregular cycles, you should better get your hormone profile done ie FSH, LH, prolactin, and TSH, which may indicate the cause. However, an estrogen level of 85 is also ok. For conceiving in a natural value, your husband's semen analysis also needs to be done. And you need to know your tubal patency test. You cannot be on pills for regulating your cycles if you want to conceive. So the next step will be follicular monitoring. Once your period stops, for which you can take the help of hormones in addition to Tranexamic acid. I will suggest you consult an infertility specialist to first stop bleeding and then on day 2 of your next cycle get a hormone profile done with a basic internal scan to see the number of antral follicles. The doctor will very carefully stimulate the eggs if required and do proper monitoring to help you to conceive.
