Hi doctor,

My sister has felt a severe headache in the left side portion of the brain for two days, and the pain is not continuous, but when it starts, it starts at a point on the left side and remains for five to 10 minutes. In a day, 20 to 25 times, the pain is repeated. I consulted a neurologist here in my hometown. He suggested a CT scan, EEG, CBC, and ophthalmologist and psychologist opinion. All the reports are normal, but today, she does not feel better. Please advice. What is the main problem, and what can I do?

I have checked the attached reports (attachment removed to protect patient identity). From what you mentioned, I suspect that your sister has a migraine headache.

Is there nausea or photophobia (avoiding lights)? A migraine causes a severe pulsating headache, usually on one side, and lasts a few minutes up to a few hours. She should receive chronic treatment if there are repeated attacks.

When was her first attack? The other possible cause is glaucoma (high eye pressure).

I suggest Paracetamol 1000 mg tablet. If unsuccessful in aborting the headache, Sumatriptan should be tried. Consult a specialist doctor, discuss with him or her and start taking the medicines with their consent.
