Hi doctor,

For almost four years, I have had what started as a swelling and drooping closure of my right eye with DV, which was sudden in onset. The right side of my mouth also goes up when it happens. It was initially not painful, but now it is always present, and I have quite severe pain in the jaw and cheek. I have consulted an ophthalmologist and a neurological ophthalmologist. I have done MRI with contrast in the beginning (focused on the orbits).

It was unremarkable but picked up a sinus issue.

I am currently taking Ventolin (Pregabalin).

I understand your concern.

The swelling and dropping of the eyelid could be a local infection. Another issue to consider would be a possible partial oculomotor nerve palsy, which can be related to inflammation or local trauma. But, in this type of disorder, the onset is usually acute and improves with time.

Do you have a double vision?

Is the eyelid dropping more prominent during the evening and less visible in the morning?

This could indicate possible myasthenia gravis. In such cases, repetitive nerve stimulation should be done.
