Hello doctor,

I am experiencing a range of symptoms affecting both my mind and body. I often find myself lacking clarity in my thinking and struggling to make decisions. Even when I do make a decision, I am quickly overwhelmed by negative thoughts and a sudden jolt of fear. Additionally, I have intermittent memory issues; some days my memory seems fine, while on other days, I struggle with memory retention.

I am also very prone to irritation, disruptions in my thought processes, difficulty in thinking clearly, confusion, low mental stamina, and lack of concentration. I experience occasional slurred speech, mild depression, and severe headaches that make it hard for me to think and cause significant stress.

Despite being a spiritual person, these problems are preventing me from fully experiencing spiritual knowledge. The knowledge I gain seems to fade away after a few days. While I occasionally have days without these issues, they tend to recur after some time.

Last year, I experienced similar problems due to a B12 deficiency, which were resolved once my B12 levels were treated. However, despite taking numerous B12 supplements this time, the issues persist. I also have ongoing digestion problems, which have been present for the past five months. Although my gastroenterologist confirmed that there are no structural issues with my GI tract and prescribed medication for indigestion, there has been little improvement.

In addition to these issues, I have developed a fissure in the center of my tongue, which has grown larger over time. I also suffer from back pain, body aches, nerve pain, small sores on the side walls of my mouth, small dots on the tip of my tongue, anal itching, a burning sensation in the anus after passing stools, a yellowish coating on my tongue, low sexual interest, and premature ejaculation. I am also unable to gain weight.

These problems began last year, and despite consulting several doctors, I have not found a proper cure. One doctor suggested that I see a psychiatrist, so I am hopeful that you can offer some guidance.

  1. Vitamin B12 levels: Your vitamin B12 deficiency has been corrected. Currently, there is an excess of vitamin B12 in your body, so all vitamin B12 supplements should be discontinued.
  2. Vitamin D3 deficiency: You have a deficiency of vitamin D3, and regular supplementation is necessary.
  3. Vitamin B complex deficiency: The cracks on your tongue and in the corners of your mouth suggest a deficiency in vitamins B1 through B6. It is recommended to take a vitamin B supplement.
  4. Worm infestation: Symptoms such as anal itching, difficulty gaining weight, and a burning sensation during bowel movements may indicate worm infestation. If you have stopped taking worm medication, I suggest taking Albendazole 400 mg now, and another dose after one week (a total of two tablets).
  5. Anxiety issues: Your symptoms, including lack of concentration, nerve pain, indigestion, memory problems, negative thoughts, irritability, and indecisiveness, strongly suggest an anxiety disorder. The fluctuating nature of your symptoms (better at times, worse at others) further supports this diagnosis.

I recommend consulting with a local doctor or specialist to obtain a prescription for the following medications if you agree with this assessment:

  1. Escitalopram: One tablet at bedtime.
  2. Albendazole 400 mg: Take one tablet now and repeat after one week.
  3. A multivitamin supplement: One tablet daily.
