Hello doctor,

I am really lost at this point, so I need some guidance before doing anything. So, to begin with, it is a 4 or 5 years old problem. It started when I held a violent sneeze five years ago. After holding the sneeze, I felt the right nostril was swollen for a few months. I used to have nutritional deficiencies at the time of the incident (the sneezing episode from 4 years ago), but now I am fine. I suffered from PCOS and Type A diabetes. I have an extremely powerful sneeze, and everyone tells me how powerful the air coming out of my nose is, which is the reason why I feel my sneezing probably damaged something. Also, I used to always block my sneezes; since this incident, I have not done it anymore. I did not feel anything for a year and a half, then I started vitamin and mineral supplements and ended with neuropathy or a facial spasm on the right side of the face. I also have a terrible headache on the right side of the forehead and mental confusion. I did an X-ray, and they found something resembling a fibrous dysplasia at the left petrous apex area. I retook a scan and MRI recently, and they found a 7 mm non-aggressive lesion in the left petrous apex, maybe invading the nasal cavity. I still do not understand why I do not feel anything on the left side of my head. Also, I did not feel anything on the left side of the head when I sneeze, but now when I am breathing, I feel like the air is going everywhere and "moving nerves" on the right side of my face precisely around the maxillary sinus area.

Did you have a contrast MRI (magnetic resonance imaging)? Did you have right ear pain or hearing loss on the right side of your body? Do you have any facial numbness on the right side? Do you still have right-sided headaches and hemifacial spasms? Maybe the lesion on the left petrous area is incidental, meaning it does not give symptoms, but you have it.

Hello doctor,

For the CT scan result, the only remarkable finding is a non-aggressive left petrous apex lesion of approximately 7 mm (non-specific in nature). Nothing else to note on the CT scan or the MRI.

They never gave me a contrast for any of the MRIs I have done so far (I took 3 MRIs). I used to have tinnitus two years ago before I totally corrected my nutritional deficiencies, and now it is totally gone. I do not feel any kind of ear pain or hearing loss per se, but I have some trouble understanding and focusing since the neuropathy or nerve spasm happened. My left side (so right side if you look at me) spasms are terrible and as debilitating as the terrible cognitive issues that it is causing me. I have trouble understanding and focusing, extreme memory issues, trouble finding my words and thinking in general, and unbearable anxiety (it gives me intrusive thoughts all the time). An interesting and maybe important fact to note is when I breathe, the spasms are worse and almost paralysis-like (the upper right side of the face comprising the nostril, cheekbone and below the eye can almost stay blocked). I sometimes have trouble closing the right eye. Also, I experience spasms in the right leg and hand. And when I touch the middle of the palm of my right hand, my hand shiver, and my facial spasms are worse. The right leg also experiences paralysis, like state where it almost cannot move (especially when I am anxious).

Do you have any idea what could be the cause of that? Sensations tell me it is a cavity area around the nose, but what do you think. The Doctors I have seen are not helping much. I used to be a student, but I had no choice but to take a forced break. If you need more details or maybe the copies of the CT scan, I can provide that.

I misunderstood the laterality. So it is your left side of the face that has spasms? If that is the case, I suggest that you undergo nerve conduction studies, and you can have botox therapy on the left side of your face which has spasms. Or, if not effective, you undergo removal of the mass on the left petrous bone. For the anxiety part and headache, you can consult with a specialist doctor, talk with him or her and take Pregabalin 75 mg 1 capsule once a day at 8 PM for 14 days if tolerable, with their consent. It is both for anxiety and pain.

Hello doctor,

Everything happens on my left side (the right side when someone else looks at me). Also, I forgot to mention that I lost my sense of smell in my left nostril when I sneezed (that is probably why I felt a sort of nervous pain starting from the nostril going up to the forehead right after the sneeze). I heard about electromyogram. Is there a difference between electromyogram and nerve conduction studies? What is best in my case (since I experience facial spasms and leg and hand spasms with cognitive issues)? Do you think it is going to point out the root of the problem? I do not experience pain per se, just super violent spasms. Also, my spasms sometimes turn my head left or right (the sides of my neck are also twitching). What do you think of it?

Yes, if it is a spasm, then you can have electromyography also. If the head turns due to spasms, then another consideration is hemidystonia. Does any of you have this in your family? You can consult with a specialist doctor, talk with him or her and take medicines with their consent, Baclofen 10 mg 1 tablet twice a day while awaiting tests.
