Hi doctor,

I am 42 years old. I am a hypertensive patient with beta-thalassemia and fits. I am taking Yasmin for a long period of time. I have never missed my pill. I got my periods after four days of taking a pill and I started a new pack instead of waiting for seven days. I got spotting till ten days of my periods. I had some bad dizziness on the seventh day. I had sex on the same day. I consulted a gynecologist immediately. The doctor told me that my blood pressure is increased. The doctor suggested taking medications for blood pressure and advised to stop OCP. She put an IUD on the tenth day of my period and told me that it is safe to use it.

She prescribed me Crina-NCR 15 mg. I did not stop taking Yasmin as I want to finish the pack. After a week, I started to have bleeding again. She increased the dose of Crina-NCR 15 mg and she checked my IUD. It was a bit down and she tried to push it back to keep it in position. At last, she removed it and told me that we can put it on the next cycle. I have completed my Yasmin pack yesterday and stopped taking Crina-NCR too. Will sex had on a single day cause pregnancy? Would Yasmin takes care of it as I was having spotting on that day? Is it fine to take Crina-NCR with Yasmin? I cannot take risks regarding pregnancy and my first delivery was a rebirth for me. Kindly help.

As you were taking Yasmin (Estrogen with Progestin), then there are no chances for you to have ovulation. You have not stopped taking pills even when you started taking other medications. So there is no way to have ovulation. Your sexual exposure was on the seventh day of your period. Every woman ovulates during the fourteenth day of her period. Eggs stay alive for a day and sperms can live for three days. So there are no chances for the sperms to be present so long there. Crina-NCR (Norethisterone) is a progesterone and so you will not have any abnormalities in periods. There are no chances to have reduced effectiveness of Yasmin. Intrauterine devices are the best emergency contraception. It can be used within five days after sex. IUD (intrauterine device) makes the inner lining of the uterus unfavorable for embryo implantation. There are no chances for you to become pregnant at this age.

I have completed my course and took a break of seven days. I got my periods with a good amount of bleeding. I have started taking a new pack of Yasmin after seven days of break. This month, I got UTI, and the doctor prescribed Niftas 100 mg twice and Mirberg 50 mg once a day. I have completed five days of course and I have to take it for two days. I have Yasmin to be taken for three days. Today I have some spotting. I do not understand the mystery. Does this mean that Yasmin has lost its effectiveness or antibiotics has reduced its effectiveness? Can I become pregnant in this situation?

As you had proper bleeding, then there are no chances for you to become pregnant. Niftas (Nitrofurantoin) induces the metabolism of hormones. So the effects of the hormones get reduced. You can take a single dose of 100 mg per day and it is sufficient. When it is taken at a higher dose, the hormone levels created by the pill gets reduced transiently. But it does not indicate that ovulation has occurred. When a pill of Niftas is used, a mild fall in hormone is seen as a withdrawal of hormones and you can have bleeding. Nothing can be done and you will become fine in the next period.

Do not worry about contraception effects. Withdrawal effects of hormones can cause minimal spotting. This does not mean that there is no effect at all. The effect of hormones gets marginally reduced and ovulation is usually suppressed. You cannot get pregnant until you get ovulation. But from next time onwards, ask your doctor to give antibiotics and medicine that does not show liver metabolism inducing effects. If you take them unknowingly, you can take a minimum dose or increase the dose of contraceptive pills. For time being, do not be worried. Your history does not suggest a chance of pregnancy.
