Hi doctor,

I have been experiencing acute pain in my groin and lower abdomen while playing football. This pain started around four months back, as a result, I had to stop playing. Thus the pain was not there since last two weeks and it started today after I played football again. The pain does not get better on taking Paracetamol tablets.

Sorry to hear about your sports-induced groin pain. The most common cause of pain in the groin in males of your age and habitus is muscle, ligament or tendon strain resulting from the particular activity. This is not a serious problem and can usually be effectively treated with a four weeks course of oral anti-inflammatory medication, proper stretching before and after activity like sports and doing moderate intensity exercise until the inflammation subsides. Other less common causes of groin pain in young males include the following. Epididymo-orchitis (infection of the testis), caused by viruses like mumps. Mumps is treated symptomatically. STI (sexually transmitted infections). Non-STI (non-sexually transmitted infection) like E-coli. It typically results in very painful testicles, which can also be enlarged and swollen. It is usually treated with oral antibiotics once the diagnosis is confirmed. Testicular torsion is a medical emergency and typically presents with one testis being suddenly very tender and painful. It can also be swollen and retracts upward. The testicular torsion needs to be confirmed by ultrasound and surgically corrected immediately.
