Hello doctor,

I have been experiencing pain at the top or base part of my penis for the last two months. Last year, I suffered from a penis injury after using a tight condom, which caused me pain for a month and then erectile dysfunction for another month. While the erectile dysfunction eventually subsided, my erections are now less flexible and cannot be pointed downwards. Recently, about six to seven weeks ago, I accidentally dropped my laptop on my erect penis while masturbating, and since then, I have been feeling soreness at the base of my penis. The pain has persisted for a few weeks, but now it only occurs after I ejaculate from sex or masturbation. I have noticed that if I avoid sexual activity for two to three days, the pain goes away, but it returns for another few days after sexual activity. The pain is delayed and usually appears hours after ejaculation, not right away.

I am concerned that this problem may be permanent, and I am not sure what could be causing it. Could you please advise me on what the problem might be and whether there is any permanent damage? Also when the pain is occurring, the dorsal vein of my penis always seems to be inflamed (larger than usual, and seems like that is where the pain is coming from). I should also mention that I have been using Heparin gel topically on the top or base of the penis (start of the dorsal vein) and along the dorsal vein. This was not prescribed, I took it because I suspected I had penile mandor's disease. It definitely reduces the pain. Do you suggest I continue using it? Or should I stop using this immediately?

I read your query and can understand your concern. Do not use heparin first. Also, injury can have these consequences. First of all your symptoms are more pointing towards prostatitis than a penile issue. Prostatitis causes pain in ejaculation. Also pain while having sex can be a prostate issue. But overall you should get an ultrasound of the pelvis and a penile Doppler. Also, start using an alpha-blocker.

Thank you for your response.

I want to clarify that while ejaculating I do not experience pain. The pain appears hours after ejaculating (or the next day). The pain is also not in my prostate. It is right above my penis, closer to my abdomen. Is it possible that I injured the penis ligament? If I stop all sexual activity can this injury heal? I suspect I already have ligament scarring from the previous injury I mentioned in my query which has caused my erections to point up and become inflexible. I am worried that this new injury will produce ligament scarring, and I read that aspirin reduces ligament scarring. Do you advise I take aspirin or could that make the situation worse?

I read your query and can understand your concern. There is a possibility of an injury in the ligament. Since only sexual activity increases the pain, I would advise refraining for a month. Also, Aspirin would not be of much help. Better to get it physically examined.
