Hi doctor,

I lost one of my tubes during an ectopic pregnancy last year. But I have severe pain on the other side during my periods. What could be the problem? I have failed to conceive; what can I do? Please help.

I understand your concern.

I suggest the following,

  • Have a healthy lifestyle with yoga, exercise, and recreational activities like swimming, outing, painting, etc.
  • A healthy, nutritious diet with less oil, salt, and sugar.
  • Try to have treatment for all diseases if you have been diagnosed or treated with your husband.
  • Trying to lower your BMI (body mass index) will increase your chances of conception and sexual activity in the fertile period of your cycle, as you have not mentioned the length of your cycle.
  • Have good hygiene. Any inflammation, infection, or scar from previous surgery or infection causes tubal blockage and leads to tubal pregnancy.
  • Better to quit alcohol, cocaine, ketamine, etc., both you and your husband.
  • This recent pain needs urgent attention.
  • It is best to have a vaginal swab test for pelvic inflammation or sexually transmitted disease and follow it seriously to get complete treatment.
  • If a partner also has problems like itching, warts, or genital infection then he should be treated after strict investigations.
  • It is best to have one transvaginal scan as well to detect pathology, if any, so that it can be treated well.

I suggest Folic acid as a routine 0.4 mg daily, along with iron supplements as well (Go for a complete blood count test to rule out anemia).

Right after the swab test, it is best to take a Doxycycline tablet 100 mg 12 hourly and a tablet of Flagyl (Metronidazole) 400 mg eight hourly for seven days. Follow with blood and scan reports, and please notice every symptom if you have any so they can correlate and could be treated.
