Hello doctor,

I have been to a spa about a month ago and I had a small redness on my dick, hardly worrying. It went away after a couple of days and did not think about it, no pain, no itching, nothing. Two weeks later, I went for a lingam massage (dick massage with oil included for like an hour) and I fall asleep after it at home, did not take a shower. Well, I woke up to a complete nightmare, my dick looked horrible. I did not masturbate for two weeks and used Betadine, and it slowly went away. Then I masturbated and boom, red again.

I went to a urologist who prescribed me a cream called Travocort and told me to use it for one week and if symptoms do not get better, go for a sperm test. Well, I used it for a week, stopped it when a week was over, and it is getting red again. Also, it started to have a little pain on urinating, and it was an absolute hell to masturbate. I would go for the sperm test to see if I have an STD which I believe is hardly possible because I have not had sexual intercourse with a woman since three months (no male either), but I really wanna ignore hospitals now that the Coronavirus is absolutely going ham.

Did you have oral sex? Is there any itching, burning in urine, difficulty in passing urine, or fever? Is there any history of allergies?

This can be either an allergic reaction which gets aggravated due to some reason, or a fungal or chlamydial infection.

I would suggest tablet Avil (Pheniramine maleate) two to three times a day for three days at least, capsule Doxycycline 100 mg twice daily for 14 days, Miconazole cream locally twice daily if the Avil does not improve things.

Avoid applying Betadine locally as skin is very sensitive.

She gave me no oral sex. I do feel slight itching, uncomfortable feeling while urinating, but not horrible. But masturbation I did in the last week was hell. I do have sensitive skin, I use baby shower gel for my body too. I will get Doxycycline and Avil. Should I use the Travocort still? I used it this morning to give me some ease, but I do not want to worsen the situation. My nurse friend recommended Bepanthen cream which should help with the inflammation. I will also take better care of changing bedsheets and underwear daily, and I use two towels one for private parts. What else can I do?

Travocort (Diflucortalone valerate and Isoconazole nitrate) for another two days will not harm. Bepanthen helps only if obvious rash or excoriation is present.

You can take anti-inflammatory like Brufen 400 mg thrice daily with meals or along with Pantoprazole 40 mg once daily for a total of five to seven days to ease the inflammation and pain (post ejaculation).

I have had a talk with my cousin's mom who is a doctor and explained what is going on. I also had opinions from three sites on the matter online and two of them and my cousins mom told me I should take Azithromycin 500 mg once daily for five days. Can you tell me your opinion, please? Either Doxycycline or Azithromycin will work. If there is prostatic involvement as indicated by pain after masturbation, it may need 14-21 days course. If the Doxycycline does not work, one of the Fluoroquinolones will do, but they can cause joint pain or tendinitis in some cases.

Hi, I would like to reach you again. I took Azithromycin for 5 days 500 mg per day and still used Travocort for those days. After that, I stopped using Travocort because it hurt my skin a lot. I figured out that if I had a fungal infection, it is definitely dead by now after using it for more than two weeks. I also change bedsheets and underwear every day and use a paper cloth instead of a towel to clean my penis after the shower. I also used less shower gel in general. I started using Bepanthem Sensiderm cream after the 5 days of taking Azithromycin. I have used Bepanthem for 7 days now. There is a great improvement.

My inflammation went down a lot, and it is hardly visible now only on the skin of the penis. But after taking shower it gets worse for 15-20 minutes, and the hole in my penis still looks inflamed. Peeing does not exactly hurt, but it is not a completely natural feeling. I masturbated two times in the last 5 days. It did not exactly hurt. But I did not feel good also. I will attach my two pictures taken before 30 minutes. I started airing my penis one hour a day and it helps a lot with healing. I would like to ask two things. Can I have a possibility to have STD even after taking Azithromycin and symptoms gone down? or it just needs time to heal. If I just need time to heal, how long does it take to heal? Should I do something else on top of using Bepanthem Sensidirm, good hygiene, and airing? Thank you for your time and help me.

Your attached pictures show improvement. STD (sexually transmitted diseases) is possible only if there is an exchange of body fluids especially in oral sex, otherwise, it is unlikely to have it. Most STDs get treated with 1 gram dose of Azithromycin. If there is a chlamydial infection or prostatic involvement, it needs 14 to 21 days of treatment with Doxycycline. Since your symptoms are better, continue the same therapy. Things should improve by seven to ten days. Good luck.
