Please suggest some remedies for slight pus-type boils on the fissures.

Hi doctor,

I have had fissures or piles with slight bleeding for the past week. It does not go in when pushed inside. I am taking Cremaffin plus syrup and using Smuth cream. Seems to be slight pus-type boils on the fissures.

Please help.

I understand your concern.

The picture (attachment removed to protect the patient’s identity) does not suggest it is a fissure. I suggest it is a thrombosed external hemorrhoids or prolapsed internal hemorrhoids. The piles or hemorrhoids have two types one is external, and one is internal. I think you are receiving good treatment. But I suggest you add the following:

1. Movicol (stool softner) sachet 3.5 grams two sachets with a glass of water to improve straining at passing stools since constipation would worsen this situation.

2. Do a sitz bath where you have to fill a bathtub up to half, add some antiseptic like dettol or savlon, and sit in it thrice daily for at least ten minutes. This will cause soothing.

Plus, continue with the ointment which your previous doctors have prescribed. You can stop cremaffin (laxative). You can do the remaining for seven days. If there is no improvement, I suggest you need to see a general surgeon for possible surgical resection.
