Hi doctor,

I have been suffering from a variety of symptoms that is becoming worse for more than 10 years, and I have spent a majority of the past eight months in hospital. Various diagnoses have been put forward within the last six months, all of which have eventually been discarded. I have undergone many tests, most of which uncover abnormalities within function the organ that is being tested, but do not identify the cause. I am on a number of medications and have also been receiving psychological treatment with no improvement. I do not suffer from any mental health conditions, as confirmed by multiple psychiatrists. We are yet to see a neurologist. My family and I have been doing some research, and have come to the conclusion that a majority of my symptoms could possibly be explained neurologically. I recently underwent an MRI scan, which uncovered a 1 cm pineal cyst. This most likely provides an good explanation for some of my symptoms, the others do not seem to be related. We have been in contact with a neurosurgeon for the removal of the cyst. A large portion of my 'other' symptoms do seem to match conditions such as autonomic neuropathy or dysautonomia. Do you think that my symptoms fit those of a neurological condition(s)? If so, which one(s)? I do have a family history of Multiple sclerosis, brain cancer and dementia.

I have difficulties remembering, concentrating and cognitive functioning. I am sensitive to sound and light, have poor eyesight and visual disturbances like double vision and trouble focusing. I have tingling, burning or numb feeling in many parts of my body, usually on one side. I also have muscle jerking, twitching, pain and cramping. I have a sensitive skin and recurrent mouth and throat ulcers. I have had constant headaches that is usually felt behind the eyes. When it worsens, it is accompanied by dull nausea. i have had roughly four to five migraines per week, which usually lasts two to ten hours. Its onset is usually at night or early morning. I am easily fatigued and have menstrual issues like heavy bleeding and pain. I also face insomnia and other sleep issues. I have frequent infections and slow wound healing. I have multiple joint issues like juvenile spinal osteochondrosis and scoliosis, along with lax joints, which cause frequent fractures or dislocations. I have an internal and external snapping hip (for which a surgery is pending) and Carpal tunnel syndrome. I also have symptoms that appear to be attributed to autonomic neuropathy. I have stomach and intestine problems like constipation, diarrhea, early satiety, nausea after eating, problems swallowing food, swollen abdomen, and vomiting. Currently, I am almost solely fed through a nasogastric tube. The symptoms related to my hearty and lungs include abnormal heart rate, blood pressure that changes with position, fainting, shortness of breath, etc. I find it difficult to initiate urination, which requires intermittent catheterization. I also have leaking urine and a feeling of incomplete bladder emptying. I sweat too much or too little, have temperature instability and heat intolerance. I am also intolerant to exercise and have vaginal dryness. I have unexplained weight loss and a small pupil in one eye. My medications include:

  1. Isotretinoin 20 mg (APO-Isotretinoin Capsules) one tablet nocte

  2. Cyproheptadine hydrochloride 4 mg (Periactin Tablets) one tablet ds

  3. Propranolol hydrochloride 10 mg (APO-Propranolol Tablets) two tablets bd

  4. Carnitine 500 mg (Acetyl L Carnitine Capsules) three capsules bd

  5. Zinc sulphate 50 mg (Zincaps capsules) one capsule daily for one week, then two capsules daily for one week, repeat Multivitamin (Tablets) one tablet daily

  6. Sucralfate 1 g (Carafare Tablets) One tablet bd Ubidecarenone 150 mg (CoQ10 100 mg Capsule) One capsule bd

  7. Ranitidine hydrochloride 150 mg (blister) (Ranoxyl (Tablets) One tablet bd

  8. Omeprazole 20 mg (APO-Omeprazole Capsules) One capsule bd

  9. Melatonin 3 mg (Melatonin Tablets) one tablet notce

  10. Amitriptyline Alphaphram 25 mg (Tablets) one tablet nocte

  11. Sandomigran 0.5 mg (Tablets) one tablet nocte

  12. Paracetamol 500 mg (Acetaminophen Tablets) one tablet tds

  13. Tramadol 50 mg (Capsules): one to two caps every three hours as needed

  14. Oxycodone 5 mg (Tablets): one to two tablets every four hours as needed

  15. Ondansetron 8 mg (blister (Wafers): one to two tablets every six hours as needed

I understand your concern.

Can you possibly upload MRI (Magnetic resonance imaging) images and other reports including ophthalmic evaluation, hormonal profile, and electrolytes profile? Your list of symptoms is fairly large and does not appear to be related to a simple pineal cyst (sometimes it is only an incidental finding on the MRI). Other musculoskeletal reports (of the hip joint, wrist joint, and spine) will help in reaching some conclusions. All pineal tumors may not need surgery, but it is difficult to comment without an MRI image.
