Hello doctor,

My husband underwent a surgery for pituitary macroadenoma. He is under medication. He is taking Histone tablet and the dosage is 10-5-5 (mg). He is advised to continue this for three months. But, he has gained a weight of 22.05 lb in the last month. Also, he complains that he has severe and intolerable back pain just below the neck. After one month of leave, he has joined back work yesterday. I am very much concerned and worried. Please advise us.

Welcome to icliniq.com. I understand your concern. The weight gain could be related to a hormonal imbalance. For this reason, I would recommend checking his pituitary gland hormones again: 1. ACTH (adrenocortical tropic hormone). 2. TSH (thyroid-stimulating hormone). 3. FSH (follicle-stimulating hormone). 4. Cortisol. 5. Prolactin. 6. Free T3 (triiodothyronine). 7. Free T4 (thyroxine). A new MRI (magnetic resonance imaging) of the brain with gadolinium enhancement is necessary to investigate for possible progression of the tumor.

Welcome back to icliniq.com. Regarding back pain, it is necessary first to investigate the possible causes. 1. For this reason, I would recommend performing a cervicothoracic spine X-ray study. A spine MRI (magnetic resonance imaging) in this region would be the best test to investigate the spine of this region. 2. If chronic degenerations are the only finding, I would recommend starting Amitriptyline or Gabapentin for some months, for the chronic management of back pain. Consult your specialist doctor, discuss with him or her and start taking the medicines with their consent. 3. It is also necessary to perform a complete blood count (CBC) and inflammation tests to exclude chronic inflammation or an infection. 4. Physiotherapy and correction of posture may be needed too. Thank you for your reply. I request an emergency advice from you. My husband and I are visiting a village. We forgot to get the Histone tablet with us. Unfortunately, Histone tablet is not available here. We will be back home in two days. Please advise. Will this cause any problem with my husband's health? Sorry for taking your time. Welcome back to icliniq.com. I would not recommend stopping Histone (Hydrocortisone) abruptly, because it can lead to adrenal gland insufficiency, with serious medical consequences. I would recommend trying to find it as soon as possible.
