Skin lesions present on hands and back. What are they?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

There are skin lesions in a 29 year old female who happened to be healthy before. The lesions started to appear two weeks ago. It has now increased on hands, back and foot. There is no itching associated with it.

Please help.

Hi,

Welcome to icliniq.com.

  1. Has the mucosa been affected? Has it spread to other areas?
  2. Does she have any fever associated with it and taking any medication?
  3. Is there any history of recent travel or herpes infection?
  4. What is the change in appearance of the lesion during the course?
  5. How it started and progressed?

Thank you.

    Patient's Query

    Hi doctor,

    Thank you for your reply.

Her mucosa is not affected. The areas currently affected are hands, feet and back. No fever. Her current medications are NSAIDs as needed for migraine attacks. It has started appearing two weeks ago. There is no travel history. It started as small lesions on the hands and then progressed to the feet and back. They started as a pinpoint and got bigger. Thank you so much for your help, as this is really concerning me.

    Hello,

    Welcome back to icliniq.com.

There are two possibilities for her lesions, pityriasis rosea, or erythema multiforme. Both are acute diseases and are usually symptomless. The good news is that both are self-limiting. A physical examination is needed to confirm the diagnosis. You may take the following medications:

    1. Tablet Prednisolone 20 mg once daily in the morning for 10 days only.
    2. Tablet Azithromycin 500 mg one tablet daily for five days.
    3. Tablet Levocetirizine 5 mg once daily.
    4. NMFE lotion (Aloe Vera and Vitamin E) is to be applied twice daily.

    Thank you.
