Hello doctor,

I am 17 years old and weigh 145 pounds. I have missed my period. I was on Depo for a year 11 years back and stopped taking it after a year. I did get one additional shot later on after one year, but I did not go back for another shot. Now I am wondering if I could be pregnant.

I read your query and can understand your concern.

Depo-Provera injection (Medroxyprogesterone acetate) effects would have worn off by now. Usually, it lasts for three to four months. But I see the possibility of no ovulation in your case, likely due to your weight.

From your history, I presume you have PCOD (Polycystic ovary disorder). This is a congenital condition, I mean it is a genetically programmed condition, you get it from your parents. When you suddenly put on weight, the excess cholesterol is converted into estrogen hormone. You might have seen obese men developing breast and young obese girls, who develop periods at a younger age. They all have high levels of estrogen hormone (female hormone).

In your case, excessive fat, which is a store for tomorrow, is more than normal leads to an overproduction of estrogen, which disrupts the balance between FSH (follicle-stimulating hormone) from the brain and the ovarian hormone. FSH as the name suggests stimulates the follicle from the ovary, but once the follicle is big enough to ovulate, the FSH is suppressed by the same estrogen hormone from the ovary. But when the same estrogen hormone is comes from fat, the brain is confused and follicle growth stops early at a smaller follicle size and will not rupture, during the scan you will see all these follicles as polycystic ovaries.

Polycystic ovaries are usually associated with obesity, irregular cycles, no periods for a few months, and later heavy periods with clots and fleshy masses, spotting on and off, thyroid abnormality, and prolactin problems indirectly. You may develop an extra pad of fat around the mid-segment of the body, especially the waist, thighs, and breasts, with no fat beyond the elbow and knees. Other symptoms may include excessive body hair growth, hair fall, a dark shade over the lower half of the face, acne, oily face skin, black skin over the back of the neck, inner thighs, and under the surface of the breast. There may also be a family history of diabetes, particularly on the paternal side.

This will not allow ovulation to occur at a time. So you cannot get pregnant, till treated. It also increases your chances of early pregnancy abortions. But this has a solution.

Your weight has to be

Weight(in Kgs) = Height (in cms)-100

Your expected weight is 114.64 to 125.66 pounds

Once you reduce weight to the normal range, you will not need any medicine to get periods or to get pregnant and pregnancy will be healthy.

To reduce weight -

1. Avoid fasting, junk food, fast foods, or red meat, and reduce snacks in between meals.

2. Have low-calorie food and high proteins.

3. Regular exercise (especially for the waist and hip area), aerobics, yoga, brisk walking (swing your hands well), try to reduce two to four kgs per month. Do not hurry on weight reduction, it will be difficult to maintain. As soon as you reduce at least 11.02 pounds you will see changes.

Another possibility is your age. Until 19 to 21 years of age, the hormone axis between the brain and ovary is not fully matured, so ovulation does not occur regularly.

I advise you to have a home urine pregnancy test, once confirmed negative have any of the oral birth control pills (Packet of 21 pills). Have one tablet twice a day for 10 days (throw away the 21st pill and inactive pills if any) no matter what the cause is (except pregnancy) you will get periods within seven to 10 days.

Due to your age, weight, and Depo injections, your hormone balance is disturbed, so regularize it, have the same birth control pills one tablet at night from the first day of bleeding for three to five cycles. During this time have a go at the weight.
