Hi doctor,

I smoked weed for about four years and had masturbated for about five years. I recently quit everything, and I feel like I have a loss of libido and struggle in getting an erection and maintaining it, and have premature ejaculation. I have this problem for about five months. I have started using Aswagandha syrup. Kindly guide me.

I can understand your concern about your symptoms. You might have erectile dysfunction (ED).

It can be due to the following reasons:

1) Depression.

2) Varicocele.

3) Fear of failure. Having one episode of ED once may make you feel you will end up with ED.

4) Low self-esteem (chronic masturbation).

5) Homosexuality.

6) Religious constraints.

7) Smoking and alcohol.

8) Long-term illness like cardiac disease, thyroid disease, kidney disease, or any other disease with long-term medication.

9) Negative attitude towards your partner.

10) Hormonal issues like a thyroid issue, low testosterone, etc.

11) Long-term drug usage.

12) Diabetes and hypertension.

You need to check the doppler penis, USG (ultrasonography) of pelvis and scrotum, blood pressure, RBS (random blood sugar), free T3 and T4, testosterone, and TSH (thyroid-stimulating hormone). Please share more details regarding the above reasons. We can work on diagnosis and treatment.

For a temporary solution, I suggest using tablet Mildfil (Tadalafil) 5 mg twice a day and tablet Duralast 30 mg (Dapoxetine) 30 minutes before intercourse. This premature ejaculation prescription will be significantly helpful for you since Dapoxetine improves the time it takes for ejaculation and the overall ejaculation control.
