Patient's Query

Hello doctor,

I developed rashes with black dots in the center, which became dry on scratching and bleed. Tiny dots on my stomach, which I think happened because of a dirty bed sheet or blanket, I used Betamethasone and Terbinafine ointment separately over each rash spot. It got cured, but due to my negligence of using the same bed sheet or blanket, no idea it again showed up with black marks on the stomach. So I once used an anti-fungal body wash with tea tree oil and neem in that, so the dryness and black scab shed off, but the tiny red dots appeared. Now I am applying only Calamine and liquid paraffin lotion by which redness has reduced, so do I need to use anything else or continue using Dermacalm lotion? I used only once an anti-fungal body wash. Can I use normal body wash? I thought of fungal infections alone, so I only used it once. The first time it happened, I thought the infection had been cured, just the skin became thin because of the topical steroid application. What should I do next? The condition improved a lot after using Calamine lotion. I treated them on my own as I had no facility for a dermatology consultation.

Hello,

Welcome to icliniq.com.

I have gone through the history and images (attachment removed to protect the patient's identity) provided by your end, and I understand your concern. The flaky spots formed following the reduction of inflammation caused due to fungal infection are normal, and they usually take time to go away completely.

1 . Calamine with paraffin lotion may be continued as it helps with soothing the area and reducing the chances of dehydration. It may be used once in the afternoon daily.

2. Antifungal body wash may be avoided.

3. Oilatum soap (Sodium palmate, Sodium palm kernelate, Paraffinum liquidum, Glycerin, and Geraniol) may be used once daily for bathing. They help with pH balance and helps with hydrating the skin.

4. Logifeel cream (Glycolic acid, Lactic acid, and Urea) may be used once daily at night on the affected area and may be continued for eight weeks or more.

5. Oilatum emollient three caps full (the bottle cap) may be mixed in 8 cups of water and used as an after bath wash after bathing. They can be used for washing body after a bath to keep skin more moisturization

6. Candid 1% (Clotrimazole) cream may be applied in the morning on the affected area to prevent secondary infections and may be used for eight weeks.

7. Capsule Omega 3 once daily at night after dinner for two months. It helps with hydration and makes skin better. Consult your specialist doctor, discuss with them, and take medications with their consent.

8. Make sure to keep the skin from the stagnation of sweat.

9. Consider drinking two and a half liters of water daily.

These steps would help with the current concern and with consistent usage, will help keep skin moisturized.

I hope I have been able to resolve your concern.

Thank you.
