Hi doctor,

I am a student, and I am 23 years old. Five days ago, I had a severe vertigo feeling (severe headache, moderate fever, nausea, vomiting, loss of balance). This symptom started every evening, and after sleeping, I was totally fresh in the morning. Here, due to the corona pandemic, I could not go to the hospital. I have a chronic history of infection in this (right ear). I had no vertigo in the past. But have a history of fungal and pseudomonas infection. I was prescribed Otosporin a lot. Eardrum was fine according to previous diagnoses. I used a drop for two days feeling vertigo are not there now, but still, some dizziness is present. And there is a continuous ringing and sometimes pain and allergy in my right ear, and the left ear is fine.

I understand your concern. Is your ear discharging frequently? As per your complaints, it is suggestive of chronic supportive otitis media(CSOM). Are you sure your membrane is intact and normal? Have you seen an endoscopic picture? I understand you cannot go to a hospital now. So, for the time being, start yourself not the following medication: 1. Tablet Amoxicillin (Amoxil) 500 mg twice daily after food for five days. 2. Tablet Montek LC (Levocetirizine and Montelukast) once daily before sleep for seven days. 3. Tablet Pantop DSR (Pantoprazole) once daily before breakfast for five days. 4. Ciplox D (Ciprofloxacin) ear drops thrice daily 5. Tablet Betahistine 24 mg (Betavert). Keep your ear dry at all times. Get an endoscopy done for the eating and get the discharge cleaned at the same time once you have access to an ENT (ear, nose, and throat) doctor.

Investigations to be done

Endoscopic ear examination, an x-ray of both the mastoids.

Differential diagnosis

Otitis externa.

Probable diagnosis

CSOM (Chronic suppurative otitis media).

Preventive measures

Keep your ear dry.
