Hi doctor,
I have chronic lower back pain for over three months, mainly from the right-hand side of the SIJ ligament and lower central spine. Although often radiates to the glutes and entire lumbar. Waking the second half of the night with pain. Stiffness and pain every morning. If I stand or sit for a long period, it is unbearable. Previous ligament strain or tear happened four or five years ago. But I have been fine over the past four years. I suspect possible SIJ ligament sprain or possible sacroiliitis, possibly due to repetitive strain from my work. I am in construction, and I am constantly bending, lifting, and twisting with weight. Blood tests from the doctor all clear, and I had a private MRI yesterday. I want help with any diagnosis and to see if there is any evidence on the images. I am attaching the test results for your reference. Kindly give your opinion.

Based on the images provided (attachments removed to protect the patient's identity) of your pelvis, I cannot see any evidence of inflammation at the sacroiliac joints at present. Minor degenerative changes are seen which are age compatible. The pelvic bones appear unremarkable. There are other causes for pelvic pain, such as chronic constipation, myalgia, etc., but these are usually not picked up on MRI (magnetic resonance imaging).
