Hello doctor,

I am a 18-year-old girl who is concerned about my mental health. I feel melancholy and hopeless about the future most of the time. I can get distracted, but if it is a bad day, I cannot smile without forcing it out of politeness. I might be talking to my friends and suddenly feel like I am a burden and should be silent and stop being happy. Sometimes, if I am down, I go to sleep to avoid being conscious. I think about self-harm, risky activities, or even death some days. I am going to a counselor, but I feel incapable of expressing myself, and it is not working out. I am too scared to ask my mother for a diagnosis because she does not believe me.
