Hi doctor,

I have a skin parasite. It is a white, almost invisible bug (not a worm, it looks like some kind of mite). It is very itchy. It has spread to my children and our family friends. Ivermectin helps a little but does not resolve completely. Please help.

I understand your concern.

According to your description, you seem to have Scabies caused by Sarcoptes scabei mites, which hide in burrows made underneath our skin. There is severe itching, especially at night. Now you had used Permethrin topical and Ivermectin orally, and they are not getting any effect on mites. Sometimes, it could be that one member of the family recovers only to be reinfected by another member of the family. So we need to treat all the members of the family at the same time. Also, take precautions like not getting in direct contact with each other or any infested items till they recover completely. Ask your dermatologist to prescribe another solution called Gamma benzene hexachloride for topical use in case Permethrin and Ivermectin do not work with all the above precautions taken.
