This is really nerve racking but I noticed these growth on my shaft (fold area) that are very small and round. They tend to itch quite often especially when touched. I can attach an picture.

I see that the growth over the shaft of your penis has been disconcerting to you (attachments removed to protect the patient's identity). It has a scabies look, which is caused by a small mite. For confirming the diagnosis, I need to know how long are you having this. Do you have itching over any other area in your body? Do any other people in your home have any recent itchy lesions over their bodies? Are you sexually active, and if so, does your partner have any genital lesions like white discharge, etc.? Thank you doctor,

I last had sexual encounter with a female escort earlier this month. We kissed, rimmed her anus and I used condom for vaginal sex. I have attached a picture which seems like some sort of tags and there seems to be two. I am not sure how long I had this for, as I did not look at it, but itching was there for as long as I can remember.

Thanks for sharing that picture (attachment removed to protect patient identity). In this, it has a fungal look. Because you say you have itching for a long while along with such scaly lesions, I would suggest you apply an antifungal cream over it first- Terbinafine cream daily twice for two weeks. I have given the generic name only. For itching, you can take a Levocetirizine tablet 5 mg daily night for five days. The skin tag like lesion that you mentioned would not subside with medicines, it would need a physical ablation, which is a simple procedure which can be done by any dermatologist. Please get back after two weeks.
