Are memory and concentration issues common in schizophrenia?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 35-year-old male with schizophrenia, and one thing that frustrates me is that even when hallucinations and paranoia are mostly controlled, I still struggle with memory and focus. I forget conversations, lose track of tasks at work, and sometimes reread the same thing multiple times before it sticks.

My psychiatrist says symptoms are relatively stable, but mentally I still do not feel as sharp as I used to. Sometimes I cannot tell what the illness is, what might be medication side effects, and what is just stress.

Do memory and concentration problems commonly come with schizophrenia, and do they usually improve once symptoms are stable, or are they often part of the condition long term?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and can understand the concern.

Memory and concentration problems are quite common in schizophrenia and may continue even when hallucinations and paranoia are well controlled. Many people find that they still struggle with the following symptoms, such as

  1. Focus.

  2. Attention.

  3. Remembering information.

  4. Completing tasks.

These are common despite having otherwise stable symptoms.

Most of the antipsychotics improve only positive symptoms (hallucinations and paranoia). Few improve negative symptoms, but cognitive difficulties often remain. Memory, concentration, and attention are all part of cognition, and these functions can be affected by schizophrenia itself.

Certain medications, especially anticholinergic drugs sometimes used along with antipsychotics, may also contribute to cognitive slowing. However, the cognitive effects of untreated schizophrenia are usually greater than medication-related effects.

Symptoms which can worsen memory and concentration

  1. Stress.

  2. Poor sleep.

  3. Anxiety.

  4. Low mood.

All these can further worsen memory and concentration problems, making them more noticeable in daily life.

However, there are a few exercises and games that strengthen memory. These include:

  1. Mental sharpening exercises.

  2. Games that improve memory and attention.

  3. Physical exercise.

  4. Meditation.

  5. Reducing the anticholinergic drug load will help you to improve your memory and concentration.

It may also be worthwhile to discuss your current medications with your psychiatrist to see if adjustments could help reduce cognitive difficulties.

I hope it helped with the query.

Please let me know if you have any more questions on your mind.

I would be happy to guide you.

Thank you.
