Hello doctor,

I am a 32-year-old male. I have small skin rash with shiny patches in a circle in the chest and on the back, once found on the scalp too. It itches when I sweat. I also have dandruff on the head and ear itching too it is dry and no ear wax produced. For skin, I have tried Surfaz SN and Zole F ointment before and it goes away for a few days and it comes back. Now, I am not taking any medicine for that. For ear infection, I tried Otobiotic and Myclin-O ear drops and it comes back when I stop the medication. I had peptic ulcer before, I occasionally take some Domperidone and Esomeprazole tablets that are the only medication I take. Besides that I take some supplements vitamins, calcium, magnesium, and iron.

By the given description it may be a fungal infection or seborrheic dermatitis or other conditions too. Please send a clear picture of the lesions. Please send the details like duration, treatment taken for it, other symptoms, other any diseases apart from those mentioned, etc., for clarification. For dandruff, avoid hard water for hair wash. Wash hair once or twice per week. Use shampoo like Scalpe shampoo twice a week.

I have attached an image of the lesion. Treatments taken are Zole F, Surfaz-SN ointment. Surfaz-NS had no effect at the same time Zole-F showed some result while taking it but when stopped, it comes back. I had peptic ulcer before and occasionally I take Domperidone and Esomeprazole tablets. Besides, I take some supplements vitamins, calcium, magnesium, and iron.

I have seen the attached image (attachment removed to protect patient identity). Continue treatment for dandruff as suggested earlier. It may be due to fungal infection or seborrheic dermatitis. Please clean the area with warm water twice daily. See that there is adequate ventilation. Apply topical antifungal cream like Terbinafine (Sebifin or Terbin) twice daily. You can use dusting powders to avoid sweat. Consult your dermatologist for examination and management if it does not subside.
