I just received a CT scan reading, and I am concerned about the first two notes.

Impression:

1. Diffuse decrease in density of the liver is seen.

2. Likely postcholecystectomy status is noted.

Well, the diffuse decrease density of the liver is a subjective finding which means it depends upon the reporting radiologist. The liver density is compared with the spleen (another organ with a similar density as of liver). I do not think that this is a significant finding alone. I want you to check liver function tests (LFTs) and show me the reports when available. I would be in the best position to tell you what you should do then.

Postcholecystectomy means after gallbladder removal. Did you have the removal of the gallbladder previously? If yes, then this statement is not a trigger for any concern. Radiologists do report the post-surgical status.

There are few more things which concern me.

Was the indication of this CT scan rectal bleeding? I want you to elaborate on your symptoms?

Secondly, the CT (attachments removed to protect the patient's identity) reports a leiomyoma of the uterus, which needs further evaluation with an ultrasound of the pelvis.

Hello doctor,

My gallbladder has not been removed. A few months before, I was not feeling well, and I went to the bathroom a few times with diarrhea. There was quite a lot of blood. I went to a gastroenterologist, and that is why she sent me for a CT scan. After that episode, this has not occurred again. I notice that my stools are soft like diarrhea, and it is frequent. The gastroenterologist prescribed two antibiotics, Metronidazole 500 mg one tablet two times a day for five days and Ciprofloxacin 500 mg one tablet two times a day for seven days. The last day I took them, I had extreme stomach pain and had to go to the emergency room. I have never had stomach problems. I do not know if the stomach ache was related to the antibiotics or to something else. The gastroenterologist gave me these antibiotics because she said that my lower abdomen's left side was inflamed. And preliminary diagnostics was diverticulitis, and later she said she would do a colonoscopy and an endoscopy. She said that for her to start any procedure, the inflammation has to improve.

Strange to hear that the radiologist misinterpreted the finding.

Well, you should discuss with your physician to correct this in the report. The stomach pain may or may not be due to the antibiotics why am I saying that is because Metronidazole (antibiotic) is known for gut-related side effects such as abdominal cramping, metallic taste of mouth, constipation, or diarrhea. But since you have per rectal bleeding lately, and the cause of which is still not known, that may be the cause of this sudden onset abdominal pain. The best approach is to go for an endoscopy and colonoscopy since these procedures would help us establish the cause for your per rectal bleeding and now abdominal pain. The above two procedures can also explain altered bowel movements such as loose stools in you.

Meanwhile, if you feel that kind of pain again, take

Capsule Esomeprazole (proton pump inhibitor) 40 mg half an hour before meals.

Tablet IB- calm (peppermint oil) thrice daily. If this tablet is not available, then try to purchase a peppermint oil liquid bottle from the store and take two tablespoons full thrice daily for pain. You can take it for many days, like one to two weeks, based on your symptoms' duration.
