Hello doctor,

My mother's stomach is severely paining from the last 4 days. Looks like bloating but it is continuous and painful. She is not pooping properly and the gas is not coming out from downside. It is coming in form of burps. The pain shifts from left side of stomach to right then to top and sometimes the whole. Her Blood Pressure is also high. Her age is 53.

I can understand that you must be very upset about your mother's health. Well, I may not be able to confirmly tell you the exact cause of her problem given limited information. But here are some possibilities:

1. Peptic ulcer disease. They are ulcers in the stomach and small intestine.

2. She might have underlying cholesterol or diabetes problem which is causing slowing of stomach.

3. If she had prior surgery for any reason in past which has now caused intestinal obstruction with unable to pass wind or poop are suggestive findings.

4. There is a possibility of other systemic problem like kidney issues, thyroid gland issues or diabetes.

Weakness possibly secondary to underlying systemic problem. For now you can give her tablet Metoclopramide 10 mg (anti-emetic)thrice daily half hour before meals plus capsule Esomprazole 40.mg (proton pump inhibitor) half an hour before meals. I suggest you to get the following blood tests which can help to approach the problem. But I highly urge you to provide me other relevent information preferrably via phone or video consultation; before getting her tested. Because none is comparison to a proper detail history.

The Probable causes

Peptic ulcer disease, gastroparesis, small intestinal obstruction, chronic kidney disease, mesenteric ischemia.

CBC TSH Renal function tests Liver function tests Urine detail report Serum CRP Serum lactate levels Calcium, Mg, Phosphorus X-ray abdomen
