Hello doctor,

My fiance had terrible stomach pain at night and in the middle of the night. The symptoms are a sharp pain in the lower left stomach and the uterus area. Sometimes it is relieved by gas, a bowel movement, or a heating pad. However, her periods are getting worse. This pain has been going on for about a year now, and it is becoming more frequent. She consulted a gastroenterologist and said she might have IBS-C, but her symptoms do not match. What could be the reason for the stomach pain? Does she need a colonoscopy? Kindly help.

I understand your concern.

There are two issues, one related to the gut and another to her reproductive system. First, regarding gut, diverticulosis is rare in young people, usually a medical condition in the elderly. It is important to ask about the type of pain, like constant dull pain, and whether there is any improvement with passing stools or when she is not having her periods.

IBS (irritable bowel syndrome) is common in young people, but before labeling it IBS, baseline investigations, stool workup, and colonoscopy are necessary. Secondly, gastrointestinal issues do not explain the change in her periods; a gynecologist must evaluate this. Finally, there is a condition known as endometriosis, in which the patient suffers severe abdominal pain, especially during or around her periods. Another possibility is to rule out UTI (urinary tract infection) and any renal issues like small renal stones, etc. I would suggest the following:

  1. Take anti-spasmodic for pain such as tablet Colofac (Mebeverine hydrochloride) 135 mg for pain.

If she has a history of constipation, it should also be treated.
