Hi doctor,

I am a 20-year-old male who is 5.5 feet and weigh 114 pounds. My problem is that I have been experiencing a health problem, testicular pain. Honestly, I have never had a vasectomy, but one of my testes has a lot of pain. Please help.

I understand your concern.

Possible causes for this pain could be:

1. Mild torsions in the testis. If you do longtime standing work, the vessels of the testis become tortuous, and they make the testis undergo torsion each time blood flows in that side. This causes sudden pulling or throbbing pain due to no cause. To avoid this, use tighter or supportive underwear, and do not allow the testis to hang down. Avoid standing for long. If you are overweight, try to reduce your weight. Usually, the pain is more on the left side. If it suddenly appears, gently hold the testis with your hand and gently pull it down. You can take painkillers. They only give symptomatic relief. If this pain is not relieved, have the testis examined by a doctor, scan of the testis or scrotum. Look for any hydrocele or any mass or significant grade 4 varicocele. These need surgical correction.

2. Urinary tract is very close to the genitals. So urinary tract infection will spread to the testis and adjoining epididymis. Pressing just above the testis and gently on the testis causes severe pain. If sexually active, you will have no control over ejaculation or premature ejaculation, a burning sensation while passing urine after sex. This can be confirmed in semen analysis. There will be pus cells and a lot of immotile sperms with morphology problems. This needs a long course of antibiotics. I suggest you take tablet Levofloxacin 500 mg once a day for 14 days followed by capsule Doxycycline 100 mg twice a day for 14 days after consulting your physician. These are prescribed for so many days, as it takes a long to flush out the bacteria from the deeper layers of the testis. If treated half-heartedly, they will recur.
