What causes testicular swelling and low attraction to women?

I have a problem with my testicle. Four years ago, I experienced nightfall, and in the morning, I noticed that my right testicle was slightly larger in length. I visited a surgeon who prescribed medication and assured me that it was not a significant issue and would resolve it on its own. However, even after four years, there is still some swelling. Recently, I consulted another surgeon who performed a color Doppler ultrasound and asked if I experienced any pain. I mentioned that I sometimes have slight pain.

The surgeon diagnosed it as mild acute orchitis and prescribed medication. After taking the medication for five days, I went back to the surgeon for another Doppler examination. This time, the surgeon stated that there was nothing wrong and that my testicle was fine. The doctor suggested that the swelling was due to anatomical reasons and ruled out orchitis or torsion. However, I did not have this swelling for four years, and I can feel the swelling in the testes when I touch it. The right testicle also feels heavier and seems to be pushing downward. Additionally, I have been experiencing a change in my feelings towards women. Instead of finding them attractive, as I did previously, I now lack that type of interest. These concerns are causing me significant discomfort and fear. Please help me regarding this matter.
