Hi doctor,

I have done two thyroid ultrasounds before two weeks. The first one predicted tr5 while the other one shows tr4. Why do I have this discrepancy?

Tr scoring system are used to interpret the findings of the ultrasound of the thyroid. The appearance and size of the nodule changes over a period of time and so it is captured in the report. I will not say it as a discrepancy but there is a change in the size of the thyroid nodule. You have not mentioned your history to suggest the condition. Please mention whether you have any swelling in your thyroid or any other symptoms.

I have attached the two reports. Do I need to send the ultrasound image? Please explain the results in detail. I have many weird symptoms for the past eight years. I have pressure in the sinus, general unwellness with paraesthesia. I have done allergy tests and they showed negative results. Nothing was found in the CT scan except the inflamed nasal passage and throat. I have consulted many doctors and taken different medications over these years. I was found with a lump in the neck before a month. I was found with another one this week. Please suggest.

Thank you for sharing the USG (ultrasound) reports. It is clear from the report that you have a multinodular goiter. It is a condition commonly seen in females where the thyroid gland develops multiple nodules. It is commonly seen in females during pregnancy. Most of the time, it remains without showing any symptoms. But sometimes, it gets enlarged and starts functioning abnormally causing symptoms due to excess production of thyroid hormones or deficient thyroid hormones. In your case, it is unlikely to have the symptoms due to thyroid.

It is important to follow your thyroid profile tests to interpret the functions of the glands to know whether it is functioning normally. You require to have a regular follow-up to see whether the gland is growing or regressing by doing USG. As it is done now, you can do another one after 12 weeks. In the meantime, if you notice any swelling in the glands or any difficulty in swallowing or breathing, then consult your physician soon without any delay. You need to do a complete blood count, TSH (thyroid-stimulating hormone), free T3 (triiodothyronine), free T4 (thyroxine), serum calcium, and serum electrolytes. Have a follow-up with the blood reports.

The first ultrasound report shows tr4 and the other one shows tr5. Why do I have these differences? I have been told that Tr shows the levels of suspicion for cancer. I have swelling in the chest and lower back. Can I have discomfort in my chest and neck due to malignant thyroid nodules? I am going to do FNA next month. Should they check only one nodule or two?

Thank you for sharing the blood reports. Your general health seems to be fine on going through the blood reports except for your thyroid profile and anemia. Regarding your thyroid profile, you have an over-functioning of thyroid gland. TR scoring helps to identify the suspicious pathology. It is not taken as a confirmative diagnosis. Thyroid FNAC (fine needle aspiration cytology) is done to see whether there is any evidence of cancerous or atypical cells. Further tests are done based on the outcome of the FNAC test. You have discomfort in the chest and neck. I do not think that they are related to the thyroid gland. These symptoms can be seen in malignant lesions where the cancer cells involve the lungs and spine. I suggest you do not think about anything till you get the results of the FNAC tests.
