I had a high fever with ear and shoulder pain. What could it be?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I had a fever throughout the night, with temperatures up to 104 degrees Fahrenheit. The fever went down to 100.5 degrees Fahrenheit this morning and is now sitting between 98.7 and 100 degrees Fahrenheit. Also, I had pain in my ears and shoulders and minor on-and-off headaches.

Please help.

Hi,

Welcome to icliniq.com.

I understand your concern.

Based on the description provided, you have a respiratory tract infection, possibly an upper respiratory tract infection, and an ear infection, possibly a middle ear.

The differential diagnosis includes -

  1. Influenza.
  2. Otitis media.

However, to reach a specific diagnosis, it is best for you to get certain tests given below.

  1. X-ray chest - AP (anteroposterior) and lateral.
  2. Blood test - CBC (complete blood counts).
  3. Ear examination.

Do you have any other complaints, such as loose motions, urinary burning, breathing distress, or anything else that you can remember, which could help in further diagnosis?

For fever, cold, and flu, I suggest,

  1. Tablet Tylenol (Acetaminophen 325 mg, Dextromethorphan 10 mg, Phenylephrine 5 mg, and Guaifenesin 200 mg) two tablets are taken at an interval of six hours for three days.
  2. Steam inhalation two to three times daily.
  3. Warm water gargles twice daily.

If you have any hospital visit records, please share them if you intend to consult again.

I hope this helps.

Kind regards.
